Continuous versus extended antipsychotic dosing in schizophrenia: Less is more

چکیده

Antipsychotic drugs temper psychotic symptoms by interacting with dopamine D2 receptors to reduce neurotransmission. Currently, the standard of care involves antipsychotic treatment protocols that achieve steady-state levels medication. Maintaining patients on continuous is thought be necessary keep them stabilised. However, exposure increases risk adverse effects over time. These include metabolic and cardiovascular disorders, extrapyramidal complications, receptor supersensitivity, latter which could potentially promote both tolerance psychosis relapse. In present review, we describe evidence showing can not only worsen long-term outcome, but—past acute phase treatment—it also unnecessary effectively manage schizophrenia symptoms. We regular but extended dosing, allowing predictable periods lower levels/D2 occupancy, safe effective in patients, it greatly reduces drug overall. Studies laboratory animals show compared exposure, dosing actually has superior antipsychotic-like efficacy, substantially likelihood motor side supersensitivity. propose regular, should considered people schizophrenia, because available suggests just as treatment, while decreasing overall reducing harmful effects.

Psychopharmacological treatment for schizophrenia: less is more

Antipsychotic polypharmacy in the treatment of schizophrenia is more common in China and other Asian countries than in Western countries. The reasons for this are unclear, but it may be related to an unsubstantiated belief among clinicians that multiple medications are more likely to achieve the desired clinical outcome. Antipsychotic medications are the mainstay of treatment for individuals wi...

Less is more: antipsychotic drug effects are greater with transient rather than continuous delivery.

BACKGROUND Most studies on the effects of antipsychotics focus on achieving threshold levels of the drug. The speed and frequency with which drug concentrations reach threshold levels and rise and fall within the day are generally ignored. Based on prior data, we predicted that variations in the within-day kinetics of antipsychotic drug delivery would produce different outcomes, even if we held...

Initial Dosing and Clinical Titration of Antipsychotic Drugs in Schizophrenia Conventional antipsychotics Dosing Optimal dosing with conventional antipsychotic agents

Conventional antipsychotics Dosing Optimal dosing with conventional antipsychotic agents involves the selection of a moderate initial dose (Table), followed by careful clinical titration to maximize antipsychotic efficacy while minimizing extrapyramidal symptoms (EPS). (If EPS develop on a typical drug, consider decreasing dose or adding an anticholinergic drug or switching to an atypical, if p...

Less is more… (more or less…).

In April 1981 Xerox introduced the Star 8010 workstation, the first commercial system with a Graphical User Interface (GUI) and the first to use the “desktop” metaphor to organize a user’s interactions with the computer. Despite the perception of huge progress, from the perspective of design and usage models, there has been precious little progress in the intervening years. In the tradition of ...

Moreover...
